NDIS Support Coordination

As your support coordinator, we will ensure that you have multiple choices of potential providers to choose. We will provide you with all necessary information about them to make this crucial decision:

  • We will compliment your life and help put your NDIS plan into action by assisting you to take the necessary steps that suit you to achieve your goals.

Home & Living Support

We celebrate the uniqueness of each person; hence we tailor our assistance to match each individual needs and goals within their NDIS plan. We cover the following personal care activities:
  • Support in taking a shower or a bath
  • Help getting dressed and getting undressed assistance accessing the restroom
  • Assistance with administering medications, as well as reminders to help you stay healthy
  • Help with chores around the house, such as cleaning, doing the dishes, washing clothes, and ironing.
  • Complex and personal care assistance through 24-hour, seven-day-a-week service
  • We support you with tasks at home, such as home maintenance, house cleaning, laundry, washing dishes and tidying your living spaces

Social & Community Participations

We match staff to participants to build rapport. This way, we can collaborate with participants to achieve their nominated goals including increasing independence and development of skills. Social & Community Participations engagement includes but not limited to the following
  • Watching sporting events
  • Going shopping
  • Sporting activities e.g Swimming
  • Visiting family and friends
  • Meeting up with friends at a café
  • Exercising e.g Going to the gym

ACME Living Care link participants goals to capacity building so that they can engage with others in the community. This helps to promote participants inclusion in community participation and employment. Such engagement and activities could be mainstream or supported activities

Supports Daily Activities

We will support you to live independently in your own home/house without being overwhelmed with everyday life struggles. Support for daily activities includes but not limited to the following
  • Appointments
  • Group Activities
  • Training and Supports
  • Shopping & Transportation
  • Running Errands
  • Companionship/Socialisation

Supports Daily Activities

We will support you to live independently in your own home/house without being overwhelmed with everyday life struggles. Support for daily activities includes but not limited to the following
  • Capacity building tasks (meal prep and cooking, cleaning, routine development, etc.)
  • We will support you with social and community participation while helping you build confidence and acquire social skills
  • We will assist you with personal care including grooming, dressing and mobility aids
  • we will support you with Household tasks such as laundry.
  • We will support you to improve your daily living skills, including assistance with exercise programs and activities
  • Support you in building and developing new skill that enhances your life
